You may place a record retrieval order by completing the Online Order Form or by downloading a form for use in a word processor.
You may also place an Order Online for a Court Reporting Job.
Some Medical Providers require a HIPAA compliant authorization before they will release their records. You may download a HIPAA Authorization for the patient to complete. LegalPartners will make every effort to let you know when an authorization is actually required.
